BOSTONAdding the drug bevacizumab (Avastin) to the standard chemotherapy regimen for non-small cell lung cancer (NSCLC) an approach approved by the FDA in 2006 did not lead to significantly increase survival rates for patients over the age of 65 enrolled in Medicare, a new study by Dana-Farber Cancer Institute researchers has found.

The study is being published in the April 18 Journal of the American Medical Association, a these issue on comparative effectiveness research.

"Effectiveness research examines the impact of treatments when they are used in day-to-day practice in ordinary circumstances," says the study's senior author, Deborah Schrag, MD, MPH, of Dana-Farber. "First, we found that bevacizumab was not universally adopted after its FDA approval. Second, the addition of bevacizumab did not provide any substantial survival advantage when added to standard chemotherapy."

Schrag will present the findings at a JAMA media briefing at the National Press Club in Washington on Tuesday, Apr. 17, 9:15 am. A separate video commentary is available on request.

A previous trial found that adding bevacizumab to the standard two-drug treatment for NSCLC the chemotherapy agents carboplatin and paclitaxel improved survival in many patients with advanced cases of the disease, but not those aged 65 years or older, the study authors write. In the new study, Schrag and her colleagues measured survival rates in patients who received bevacizumab-carboplatin-paclitaxel therapy and in two groups of patients who received carboplatin-paclitaxel therapy one group receiving it in 2006-2007, the other in 2002-2005.

They found that the median overall survival was 9.7 months for patients receiving the three-drug combination compared with 8.9 months for the 2006-2007 standard-therapy group, and 8.0 months for the 2002-2005 standard-therapy group. The probability of surviving for one year was 39.6 percent for those who received the three drugs vs. 40.1 percent for the 2006-2007 standard-therapy group, and 35.6 percent for the 2002-2005 standard-therapy group. Controlling for factors that can skew the results, the authors did not find a significant difference in overall survival between patients treated with bevacizumab and those treated only with standard chemotherapy.

Based on these results, the authors recommend that bevacizumab should not automatically be administered together with carboplatin-paclitaxel for older patients with NSCLC. They continue that clinicians should exercise caution in making treatment recommendations and use bevacizumab judiciously for their patients.

The authors also advise that for cancers like NSCLC that tend to arise in elderly patients, clinical trials of potential new therapies should include an adequate number of older patients and ensure that data for these patients is analyzed.

In Alzheimer's disease, brain neurons become clogged with tangled proteins. Scientists suspect these tangles arise partly due to malfunctions in a little-known regulatory system within cells. Now, researchers have dramatically increased what they know about this particular regulatory system in mice. Such information will help scientists better understand Alzheimer's and other diseases in humans and could eventually provide new targets for therapies.

In a study released online in the Proceedings of the National Academy of Sciences Early Edition this week, the team at least doubled the number of proteins found to be subject to a type of regulation based on a sugar known as O-GlcNAc (oh-GLIK-nak). The O-GlcNAc system likely adds another layer of control to the proteins that serve as a brain cell's widgets and gears -- control that might be muddled in Alzheimer's brains known to have problems in sugar metabolism.

"We found many novel proteins providing insights into new aspects of cell biology," said analytical biochemist Feng Yang of the Department of Energy's Pacific Northwest National Laboratory and lead author on the study. "We think O-GlcNAc is fine-tuning cellular processes."

In addition to finding hundreds of proteins modified by O-GlcNAc, the team found that almost all the O-GlcNAc proteins were also subject to the most common form of protein regulation, which uses small phosphate molecules to turn proteins on and off. This suggests a larger coordination between the two regulatory systems.

"These results show there's a level of complexity about how biology operates that we've been largely blind to," said PNNL's Richard D. Smith, who leads the proteomics team at PNNL. Proteomics researchers try to understand how a cell functions based on the numbers and types of its proteins at work, which are collectively known as the proteome (PRO-tee-ohm).

"Back during the Human Genome Project, we asked, how could so few genes produce the complexity of an organism or even a single cell, and how could minor variations in our DNA explain the diversity we see all around us? Clearly the proteome is the answer," said Smith.

Sugar Switch

Proteins are the tools, gears and gadgets that run a cell. Regulatory systems within cells turn proteins on and off by attaching or detaching small molecules to the proteins, like a switch. The most common switch involve adding or removing phosphates, and biologists have known for a long time that these switches can run amiss in cancer and other diseases. Drugs affect players in the phosphate regulatory system to try to fix the errors.

A couple decades ago, researchers found that O-GlcNAc, a kind of sugar, could also work like a switch, turning proteins on or off. Scientists found proteins decorated by O-GlcNAc, as well as other proteins that attach or remove the sugar -- all essential parts to the system.

But they had trouble finding enough O-GlcNAc proteins to get the whole story. Few proteins bore the small sugar, and those that did tended to lose the accessory while being manhandled in the lab. Researchers could make up for some of these problems by starting with more tissue or cultured cells, but they knew if they wanted to look for these modifications in real-life scenarios such as clinical samples, they would need to be able to find the sugar with a small amount of starting material.

To overcome these difficulties, Smith, Yang and their colleagues at PNNL and four research institutions combined their expertise in the O-GlcNAc system with instruments developed at EMSL, DOE's Environmental Molecular Sciences Laboratory on the PNNL campus. First they improved how they purified protein from mouse brain tissue to reinforce the sugar attached to proteins. Then they used instruments that exceled at detecting rare proteins in small samples.

In addition, they looked for the sugar-dotted proteins in mouse brain samples from engineered animals that had a mouse version of Alzheimer's. These mice make too much of three key proteins implicated in Alzheimer's disease in people, including the Tau protein, which forms the hallmark tangles in brain neurons.

Pack o' Proteins

To test how well their methods found O-GlcNAc proteins, the PNNL-led team started with tissue from either healthy or diseased mouse brain tissue. From the healthy tissue, the team found 274 different proteins marked with O-GlcNAc. Many of them sported more than one sugar molecule, because the team found a total of 458 attachment sites on those 274 proteins -- triple the number of sites found in any previous study. The large number of sites allowed the team to identify similarities between O-GlcNAc sites, as well as O-GlcNAc sites on previously unexplored proteins.

Of the 274 O-GlcNAc proteins, 106 had already been identified in other studies. That left 168 newly-identified proteins. Based on what the proteins looked like, the team classified most of them as likely being involved in cell signaling, regulating how genes are expressed, or, again, in cell scaffolding.

The O-GlcNAc-dabbed proteins held a variety of jobs, including forming part of a cell's scaffolding, or in nerve growth or in other nerve-related occupations such as learning and memory.

The PNNL-led team then looked at the proteins found in the Alzheimer's-like mouse brain. They found about a third fewer O-GlcNAc-marked proteins. That result also supports earlier work that suggested there is damaged O-GlcNAc regulation in Alzheimer's brains in people.

Fraternizing Phosphates and Other Biology

One of the more exciting things the researchers found had to do with the most common regulatory system in cells, the phosphate system. More than 98 percent of the O-GlcNAc proteins also had sites that would accept a phosphate, suggesting those proteins are also under the control of that system.

And about a quarter of the O-GlcNAc sites were close enough to the phosphate sites to interfere with that switch, suggesting cross-talk between the two types of regulation. A phosphate is smaller than O-GlcNAc and has a strong negative electrical charge. The sugar is neutral but bulkier. Those characteristics could have different effects on the structure of the protein and greatly increases the range of possible biological effects due to the complexity of the combined switching systems.

Lastly, until this study, most of the proteins known to be under O-GlcNAc control largely live their lives within the cells. But the PNNL-led team found a half-dozen proteins that had to be controlled by O-GlcNAc outside a cell, based on where their O-GlcNAc site fell on the body of the protein.

Now, the team is planning to measure both regulatory systems in concert.

"It's revealing to see how many proteins are modified. If we're going to understand biological systems, we need to understand the interplay of the different types of modifications," said Smith.

Apple says e-book price fixing charges
Not that we were expecting Apple and Macmillan to simply fess up and say, "you're right, totally tried to circumvent the free market," but both companies have come out swinging pretty hard against the allegations of price fixing. Apple has rejected the charges, calling them "simply not true." A company spokesman, Tom Neumayr, went so far as to tell Reuters that Cupertino was actually fostering competition by "breaking Amazon's monopolistic grip on the publishing industry." John Sargent, the CEO of Macmillan, defended his company's behavior in blog post, saying the publisher had done nothing illegal and that the concessions sought by the DOJ in settlement negotiations were "too onerous." It looks like the next step for both is to face off with the US government in court -- a daunting task, no matter how large your war chest.

The success of "Titanic" is undeniable. With a worldwide gross of $1.8 billion, the film is the second highest grossing movie in box office history — just behind director James Cameron's other massive project, "Avatar." The historical film was nominated for fourteen Academy Awards and won eleven, including Best Picture, Best Director, Best Cinematography and Best Original Song.

It also launched the careers of two of Hollywood's most well-known actors, Leonardo DiCaprio and Kate Winslet.

The 3-D re-release (which raked in an additional $17.3 million during its debut last weekend) brought rumors to the surface that other popular actors had turned down the role.

When MTV News' Josh Horowitz sat down with Cameron, Horowitz asked about the rumored "Titanic" casting circulating the Internet. Supposedly, Matthew McConaughey was in talks for Jack Dawson and Gwyneth Paltrow for Rose DeWitt Bukater. "I will neither confirm nor deny," Cameron said. "I just don't think that's cool to talk about actors that either chose not to do it, or were unavailable, or stupidly decided that there wasn't enough meat on the bone of the character, or whatever it was."

We bet those actors came calling back begging for another chance. "They should," the award-winning director said. "They should've come whimpering back afterwards and said, 'I will never second guess you again as long as I live.' But it's too late. You got one chance, that's it. And you might want to think about this next time when you get called." And even though the role of Rose follows Winslet everywhere, there's no doubt it was the experience of a lifetime. She recently told us that she's glad the film can reach a whole new audience, including her children who will see it for the first time.

But apparently, some audiences didn't know the movie is based on a true story. After seeing the film in 3-D, people have taken to Twitter to admit they didn't realize the tragic incident actually occurred. "That's shocking to me," Cameron commented on the news, "that somebody could actually tweet — because it requires some neuromotor capability — and not know that Titanic was a real event, but apparently there are [those] people."

"It's a sad comment on our society. However, it does put the onus on us to get the history right. Because it might be the only record a thousand years from now," the director laughed, "the one disc might be the only record."

MagentoLast year, eBay eBay acquired Magento, developer of an open-source e-commerce platform for around $180 million. Magento became part of X.commerce, a combined open platform business from eBay and PayPal. A year after the acquisition was announced, Magento co-founder and CTO Yoav Kutner has left eBay. While fellow Magento co-founder Roy Rubin sent Kutner off with a warm farewell, Kutner doesn't have such nice things to say about eBay and X.commerce, according to this post on Quora. In response to the question of why he left eBay, Kutner responded (Kutner confirmed to us that he posted this response): I will answer and give much more information over the next few weeks as to the reasons I had to leave Magento. As for now I can say that (very) short term I would not be worried about the 'open' part of Magento, but as I have learned eBay and the folks at X.commerce don't really understand the meaning of open and have a hard time explaining and defining it to them selves and to others. As such, long term, it would be very interesting to see if Magento will continue to stay open in the manor the people behind Magento and I meant it to be.

Linebacker Lance Briggs wasn?t happy with his prior contract.? He now has a new contract.? Presumably, he?s happy with his new contract.

In light of the veteran linebacker market, he should be.

Per a league source, Briggs? new deal pays $17.5 million over three years, $5 million of which is fully guaranteed.? (As a practical matter, another $3.25 million is guaranteed, given that it?s highly unlikely the Bears will cut him this year.)

In 2012, Briggs gets $3 million to sign, a $3 million base salary, and a $250,000 workout bonus.? That gives him $6.25 million; under his prior deal he would have earned $4 million.

Next year, Briggs is due to receive a $1 million roster bonus in June and a $4.5 million base salary, $2 million of which is fully guaranteed.? He also is eligible for a $250,000 workout bonus.? It?s a payout of $5.75 million.? Under the prior deal, he would have earned $6.5 million in non-guaranteed base salary.

In 2014, Briggs is due to receive a $500,000 roster bonus in June and a non-guaranteed base salary of $4.75 million.? He also is eligible to receive a $25,000 workout bonus.

Though the deal doesn?t carry much ?new? money over the next two seasons, the $8.25 million in guarantees will go a long way toward alleviating the concerns of a thirtysomething linebacker who is navigating a new regime.? Even if the Bears had paid Briggs $4 million in 2012 without squeezing him to take less, the chances of Briggs getting cut in 2013 would have gone up significantly ? and then he would have found himself dealing with a linebacker market that is almost as soft as the running back market.

So it?s a win-win for the Bears and Briggs, and it shows that new G.M. Phil Emery is willing to take care of the guys who have been with the team.

The only remaining question is when and if Emery will take care of running back Matt Forte in the way Forte wants to be taken care of.

President Obama hosted George Clooney for dinner, and now Clooney is returning the favor.

The Oscar-winning actor and longtime Democratic activist will host?a 150 seat, $40,000-a-plate fundraising dinner in honor of the President's re-election campaign at his home. The event will be co-hosted by?Dreamworks CEO?Jeffrey Katzenberg; the exec was slated to host the party, but his $35 million new home was not finished in time, which led to the search for a new location.

Clooney?sat just a few seats away from the President at the recent White House State Dinner in honor of British Prime Minister David Cameron and?has attended two other major Hollywood fundraisers for Obama in Los Angeles already this year. The news of the upcoming fundraiser was first reported by Entertainment Tonight.

Clooney will also attend the White House Press Correspondents Dinner as a guest of Time Magazine.

Obama's campaign has made a concerted effort to woo major Hollywood donors after a dip in support as compared to the industry's enthusiasm for his 2008 run. Harvey Weinstein hosted a fundraiser for Obama in New York in March, while Michelle Obama was feted at a Robert De Niro-hosted dinner. The President has flaunted some of his celebrity connections, as well, including giving Aziz Ansari a shoutout in a speech that has since become a staple of late night conversation.

Clooney, who has been vocal about his continued support for Obama, recently gave the campaign a bit of publicity advice -- he does know something about public relations, after all -- when he told the President to run on his accomplishments.

"The Democrats are just very poor, in general, at explaining what it is, when they accomplished something, I think they?re pretty bad at it, and Republicans are very good at it," Clooney told Meet the Press host?David Gregory. "If I was a Republican, and Obama was a Republican, I would be selling all of the, you know, he saved the auto industry and he got Osama bin Laden. He passed a health care bill that no one could pass -- if that was a Republican issue. I would be able to sell his presidency as a very successful one. But Democrats are bad at that, we like to pick each other apart."

Working in the personal claims industry, one of the most common questions that we get asked is: How much compensation will I receive? And rightly so. If you?re going to take time out of our day to start a compensation claim, even if 95% of the work is handled by a solicitor and claims expert such as My Compensation, you should want to know how much compensation you can expect.

There are two main factors to take into consideration when working out roughly how much compensation you?re owed. The first is how seriously the injury, insult, or other wrong-doing has affected, or will affect your life. The second thing to consider is how much the offending person is to blame for the incident. Let?s use personal injury compensation claims as an example here.

The hand is a good part of the body to use as an example to show how much compensation can be expected, as it?s made up of different parts. For example, a ?pincer? grip is when you hold something between your thumb and index finger. This is important for manipulating objects in your everyday life. Your little finger however, is arguably used less in day to day life and so, from a legal perspective an injury to the little finger would be worth less compensation than if you damaged your thumb and/or index finger.

In personal injury cases, the severity of the injury is also taken into consideration when decided on how much compensation is owed. A multiple break is worth more compensation than a hairline fracture and a cut that requires a few stitches might be worth less than a hairline fracture. To give you an example, in the UK last year the average thumb fracture compensation claim was worth around ?2,900 while the average little finger of ?pinkie? fracture was worth ?1,100.

With other types of compensation claim, a similar pattern follows in gauging how much compensation is owed. If you?ve suffered psychologically at work due to discrimination, then that would be considered a serious case and the compensation would depend on how seriously a psychologist deemed your suffering. If you were fired because of discrimination, then loss of earnings would also be compensated for. With these cases, the blame can be squarely places on someone else?s shoulders too and that is important, as you?ve nothing wrong. In a car accident claim for example, blame is often divided into a percentage. If an incident was 100 per cent the offending drivers fault, then more compensation will be paid.
